Analysis of Middle Cerebral Artery Aneurysm Associated with the Intracerebral Hematoma


Abstract
In 1990 Norman Dott seems to have been the first surgeon to have operated on a ruptured aneurysm of the middle cerebral artery. Aneurysm of the middle cerebral artery bifurcation are common, and may have a hemorrhage causing many neurological
signs
and
poor outcome.
The author have to get a analysis on the ruptured aneurysm of middle cerebral artery associated with the intracerebral hematoma at Pusan National University Hospital during a few years.
@ES The results were the follows:
@EN 1. The ruptured aneurysm of middle cerebral artery associated with the intracerebral hematoma occurred mainly after 30's, and predominatingly in female.
2. The cases had commonly been taken a cerebral angiography in this hospital within the 1st day.
3. Mainly the aneurysm was present at the bifurcation of middle cerebral artery.
4. The aneurysm was found at the right side in 25 cases and at left side in 13 cases.
5. The mass effect, vasospasm and hydrocephalus appeared by cerebral angiography.
6. Usually the size of aneurysm was within 20mm in the diameter.
7. Main location of the hematoma was in temporal lobe, and also in frontal and parietal lobes.
8. Vomiting, headache, deteriorated consciousness, and neck stiffness were found in many cases.
9. Also other many neurological features appeared in many cases.
10. At admission the neurological grade II, III, IV were found with roughly same rate in male, but severely deteriorated neurological grade, such as Grade III and IV in female.
11. Many cases had mass effect and hypertensive finding.
12. Of 42 aneurysms the surgical treatment was performed in 38 aneurysms, but not in 4 aneurysms.
13. The cases with better neurological grade had relatively good outcome, and poor cases showed bad outcome.
14. The cases with high systolic and diastolic blood pressure had relatively poor neurological grades.
15. The poor outcome was relatively with high blood pressure.
On these results early surgery for life saving and good outcome in middle cerebral aneurysm associated with the intracerebral hematoma should successfully be performed.
